2. If you have insurance We accept CareOregon and most insurance carriers. If you have a copayment, we will collect it at time of service (VISA, MasterCard, cash or local check). We will bill your insurance carrier for you and you will be responsible for the balance. If you have no insurance We offer a sliding fee scale based on income for those without insurance. Please contact the clinic for further information. 3. What to bring on your first visit - Insurance card and proof of income if applying for the sliding fee scale
