Formulary List

The Formulary List provides a directory of all the preferred medications currently approved for CareOregon members. Each formulary contains a list of drugs that are covered by CareOregon OHP or CareOregon Advantage. Use the links below to view the formularies in a PDF format.

You can search for a drug in one of the following ways:

  1. Find the drug listed in the formulary/PDF index; or
  2. In the PDF file, enter the drug name into the search box located in the menu; or
  3. Go to ePocrates, search online or download CareOregon, Inc. formularies to your handheld device. See the Download Instructions
    Select CareOregon for Medicaid (OHP) Formulary; CareOregon Advantage Plus MA or CareOregon Star MA for Medicare (COA) formulary under “Oregon” on the formulary selection page.; or
  4. Call Customer Service, at 503-416-4100 or 1-800-224-4840. TTY/TDD users can call 800-735-2900.
