New OHP applicants or current OHP recipients due for re-eligibility will be required to provide proof of U.S. citizenship. This new requirement does not apply to those receiving Medicare benefits. Examples of proof of U.S. citizenship might be a copy of your birth certificate, U.S. passport or naturalization papers.  Oregon's Department of Human Services (DHS) will help Oregon-born applicants by checking the agency's database of birth certificates, which goes back to 1903 and is available electronically from 1920. For people born in other states, DHS will provide details about how to order birth certificates and, in specified hardship cases, will pay the cost of obtaining the document. If you were born in Oregon, you can request a copy of your birth certificate through the state of Oregon by completing this Birth Record Order Form. For more info about new citizenship requirements, click here.
