What you need when you get your taxes done

You will need to have certain documents and information when you get your taxes prepared. Here is what you will need to have when you go for help to a Volunteer Income Tax Assistance (VITA) or Tax Counseling for the Elderly (TCE) location:

Tax Form
  • Proof of identification, such as driver’s license with photo, passport or state non-driver’s identification card. (You can get an Oregon non-driver’s identification card at a Department of Motor Vehicles office. For information, click to the DMV web site.)

  • Social Security Cards for you, your spouse and your dependents, or a Social Security Number verification letter from the Social Security Administration.

  • Birth dates for you, your spouse and your dependents written on the tax return.

  • Current year’s tax package if you received one.

  • Wage and earning statement(s) forms W-2, W-2G and/or 1099-R from all employers.

  • Interest and dividend statements from banks (Forms 1099).

  • A copy of last year’s federal and state returns if available.

  • Bank routing numbers and account numbers if you would like direct deposit of your return.

  • Total you paid for daycare providers and the daycare providers’ tax identifying number (the provider’s Social Security Number or the provider’s business Employer Identification Number).
