March 28 -“We want to make sure that no one falls through the cracks,” said Erin Kirk, Quality Manager at Virginia Garcia.

Virginia Garcia Memorial Health Center has implemented a disease management and registry tracking system, called MediTracks. The system helps staff identify key health information about high-risk patients.

Launched in 2005 with funding by CareOregon’s Care Support and System Innovation Program (CSSI), MediTracks is enabling Virginia Garcia to collect data for many chronic illnesses, including asthma and diabetes. full story...


Asante Wins Home Care Technology Grant

January -Asante Health System is receiving nearly $500,000 to bring tele-monitoring services to more than 500 home care patients served by Rogue Valley Medical Center (RVMC) and Three Rivers Community Hospital (TRCH).

The technology allows health care workers to remotely monitor the vital signs of their sickest patients in rural areas, because data is automatically sent to central stations at the RVMC Home Care office in Medford. full story. . .
