CareOregon’s CareSupport Program

Do you have several health issues? Do you feel overwhelmed and struggle to follow your doctor’s treatment plan? Do you have trouble making it to your medical appointments? Are you having trouble getting the extra help you need?

You can call CareOregon for help.

Our CareSupport team may be able to give you the help you need.

CareSupport’s team includes nurses, social workers, and others who help people with chronic conditions like asthma or diabetes. They can also help you with other medical issues and connect you to helpful social and community services.

Sometimes members need extra help on their road to good health.

  • They might have more than one health issue, like asthma, diabetes and chronic pain.
  • They might have problems understanding or following their doctor’s treatment plan.
  • They have trouble making it to their medical appointments.
  • They have issues like homelessness or hunger that make staying healthy difficult.

Our CareSupport Team wants to help.

Here are some—but not all—of the kinds of members we’re reaching:

  • Mothers who have recently had a baby.
  • People with both Medicaid and Medicare.
  • Pregnant women who are on methadone.
  • Patients with chronic illness like congestive heart failure, asthma, COPD or diabetes.
  • Patients leaving the hospital and going to a skilled nursing facility or home.
  • Patients in the hospital for mental health reasons.

Our team includes:

  • Registered Nurse Case Managers try to head off health care crises. They encourage and teach members, and help make sure their health care services are coordinated. They work with members and doctors so that treatment plans are more successful.
  • Behavioral Health Case Managers help members with mental health, substance abuse and behavioral issues. They really listen to patients. They help them discover behaviors that are less harmful and that improve health.
  • Health Care Guides build strong relationships with members. They schedule appointments and arrange rides to the doctor. They help members find other resources that are important to health, such as shelter, food and clothing.
  • Clinical Pharmacists help with medicines. They work with members and their doctors to find the most appropriate drugs.

Members with Medicare or Medicaid can have CareSupport help if they need it.
