  • How do I get an Oregon Health Plan (OHP) application or reapplication?

All applications or reapplications come from the state. Call 1-800-359-9517 to receive an application or reapplication. It is important to know that it is the state that approves applications or reapplications, not CareOregon.

  • How do I get information on OHP premiums?

For information about premiums and payment, call toll-free 1-888-647-2729.

  • Who do I contact about name, address or phone number changes?

Any name, address or phone number change must be given to your caseworker at your branch office. Your branch office and the phone number are located on your DMAP Medical Care ID, underneath your address.

  • Is medical transportation available?

Transportation to medical appointments is a benefit for OHP Plus members. DMAP pays for this benefit. In counties that do not have a medical transportation carrier, contact your caseworker or local Adult and Family Services (AFS) office to find out about transportation options.

  • Is interpretation available if I don’t speak English?

If you do not speak English or need sign language interpretation, it is your legal right to have an interpreter at your medical appointment. CareOregon Customer Service can help you arrange to have an interpreter at your appointment.

  • Do I have dental coverage?

People on CareOregon also have dental coverage. It is provided by a contracted Dental Care Organization (DCO) in your area. To see the name of your DCO, look on your DMAP Medical Care ID.

  • Do I have mental health coverage?

People on CareOregon also have mental health coverage. It is provided by a contracted Mental Health Organization (MHO) in your area. To see the name of the MHO in your area, look on your DMAP Medical Care ID.

  • Does CareOregon cover treatment for drug or alcohol problems?

Yes. Our drug and alcohol treatment providers are listed in the Provider Directory for CareOregon Members, which you should have received when you joined CareOregon. You can also click here to use our online Provider Search.

  • How can I get an advance directive form?

An advance directive tells your doctor what kind of care you want if you cannot make medical decisions (if you are in a coma, for example). You can authorize health care representatives to allow death to occur naturally if you are medically confirmed to be in one of the conditions described in Part C: Health Care Instructions in your completed advance directive. An advance directive does not authorize euthanasia, assisted suicide or any action to end a person's life. Click here to display and print an advance directive form.
