This online directory has names and addresses of CareOregon providers. To find providers in your area:

1. Click a provider type.
2. Click your city or county.
3. Click Search.

To see a list of CareOregon PCPs, click on the Primary Care Providers category.

To select a PCP for yourself or a family member, please call CareOregon Customer Service.

Important to Know:

• Except for vision, chemical dependency, family planning or obstetric appointments, your PCP will coordinate your care.
• This directory does not identify providers who accept new patients. For help, please contact our Customer Service Department.
