2010/2011 CSSI Program Description

Goals
The goals of the CSSI Program are to:

Foster a culture of evidence-based practice and continuous improvement in CareOregon provider organizations.
Provide training, coaching and technical assistance to CareOregon providers.
Empower providers over time with the skills, knowledge and resources to create and sustain the substantial changes necessary to transform health care.
Develop strategic partnerships with providers who share CareOregon’s mission of serving the underserved.
Develop and enhance renewal/medical home practices.

Project Categories
The categories of CSSI funding are:

Introduction to Continuous Quality Improvement
Primary Care Practices: Continuous Quality Improvement Projects
Primary Care Practices: Creating and Enhancing Renewal Practices/Medical Home Projects
Specialty Care Projects
Hospital Projects (guidelines to be released on March 26, 2010)
