2010/2011 CSSI Program Description

Goals
The goals of the CSSI Program are to:

  • Foster a culture of evidence-based practice and continuous improvement in CareOregon provider organizations.
  • Provide training, coaching and technical assistance to CareOregon providers.
  • Empower providers over time with the skills, knowledge and resources to create and sustain the substantial changes necessary to transform health care.
  • Develop strategic partnerships with providers who share CareOregon’s mission of serving the underserved.
  • Develop and enhance renewal/medical home practices.

Project Categories
The categories of CSSI funding are:

  • Introduction to Continuous Quality Improvement
  • Primary Care Practices: Continuous Quality Improvement Projects
  • Primary Care Practices: Creating and Enhancing Renewal Practices/Medical Home Projects
  • Specialty Care Projects
  • Hospital Projects (guidelines to be released on March 26, 2010)
