Medication Safety Alerts We encourage CareOregon providers to check the U.S. Food and Drug Administration MedWatch web site and sign up for the FDA Drug Safety Newsletter to receive safety alerts via e-mail. We will inform you if CareOregon takes any formulary actions in response to a safety alert. Adverse Events and Errors Be sure to report serious adverse events and medication errors. A medication error is "any preventable event that may cause or lead to inappropriate medication use or patient harm while the medication is in the control of the healthcare professional, patient, or consumer. Such events may be related to professional practice; healthcare products, procedures, and systems, including prescribing; order communication; product labeling, packaging, and nomenclature; compounding; dispensing; distribution; administration; education; monitoring; and use."
