The CSSI Program seeks to support provider organizations that identify and support teams to lead projects and initiatives that improve health care delivery and outcomes. Each project demonstrates improved design, outcomes reporting, institutional commitment and capability. Examples of projects that have been funded are improving the care of patients with diabetes and other chronic diseases, establishing primary care teams, advance care planning, reduction of health care acquired infections, Primary Care Renewal, and Releasing Time to Care.
