The utilization limitations for incontinence supplies have changed in accordance with the Department of Medical Assistance Programs (DMAP) new guidelines. Effective 8/1/2011, incontinence supplies in the HCPC range of T4521-T4535 are limited to 200 per month. Gloves are limited to 2 boxes per month.
