Effective January 1, 2012, several drugs will be removed from CareOregon Advantage (Medicare) formulary. The table includes some of the drugs being removed and formulary alternatives. We want to avoid any disruption to therapy. Current members who will be affected by negative changes will be given a 60-day transition supply during the first 90 days of the new plan year. We will also be contacting prescribers who have members affected by select changes before the end of 2011 to help them make the transition to formulary alternatives or request an exception.
In addition to these changes, some drugs now will have quantity limits or require prior authorization or step therapy. Please refer to the 2012 CareOregon Advantage Formulary, Prior Authorization Criteria, Quantity Limits, and Step Therapy Criteria.
