What is in this program?

Our trained staff at CareOregon can help you with things that may keep you from being healthy. We can connect your doctors, other providers, home and community support agencies and social service organizations. Our specialty is to work together with your care team to help answer any questions you may have and help you overcome barriers to your health.

The program also can help you:

  • Find a provider for your health care needs, including mental health and substance use.
  • Get appointments with primary care providers, specialists and other health care or behavioral health providers.
  • Get rides to appointments.
  • Get needed medical equipment, supplies or services.

Care management

If you have a serious or chronic health condition or disease, we have a team of professionals that are ready to help you.

These are conditions such as:

  • Diabetes
  • Heart failure
  • Asthma
  • Depression
  • Substance abuse (like drugs, alcohol or tobacco)
  • And more…

Our team includes a nurse, behavioral health specialist and health care coordinator. Together, we can help you understand treatment plans set by your primary care provider. We also will help make the most of provider visits and connect you with more help in your community.

Care Management teams talk with you about how to live well and feel better with your condition.
We want to help you feel positive about life.

Intensive care management

If you have multiple chronic conditions, special health care needs, or if you are disabled, you may be a candidate for Intensive Care Management services. Your primary care provider will most likely determine this, but you also can contact us if you think you need these services.
