Purpose statement: To build upon the Doctor of Pharmacy (PharmD) education and outcomes to develop pharmacist clinicians with diverse patient care, leadership and education skills, who are eligible for board certification and postgraduate year two (PGY-2) pharmacy residency training. Ambulatory Care Pharmacy Residency Program (PGY-2)

Previously our PGY2 residency in Ambulatory Care offered an opportunity for pharmacists to expand on previous experience in pharmacy practice or ambulatory care to develop transitions of care and population health management for Coordinated Care Organizations (CCOs) Columbia Pacific, Jackson Care and HealthShare as well as established primary care clinic partnerships. The PGY-2 program focus was traditionally focused on service development and business justification for services within home based pharmacy care, telemedicine, plan based operations, primary care clinic operations, data analytics and panel management in expansion of pharmacist clinical services to regions within the Portland metro area and rural partners served by CareOregon. Currently our accredited PGY-2 Ambulatory Care Residency Program will not be seeking new candidates while we evolve our internal ambulatory care opportunities to best facilitate the PGY-2 residency experience with CareOregon.
