Community partner resources

We’re happy to share the following list of resources. They focus on helping community-based organizations (CBOs) and community members learn about and navigate the Oregon Health Plan (OHP). Most pieces are available in multiple languages.

CareOregon believes that a strong network of providers from community organizations is indispensable in providing access to health-related and social services. We’re grateful for their partnership.

Overview

We know how confusing the Medicaid landscape can be, so we worked with members to create an overview of how the Oregon Health Authority (OHA), coordinated care organizations (CCOs) and providers are connected.

If you are eligible for both Medicaid and Medicare (which means you are "dual eligible"), we want to make sure you understand the differences between them and how they can work together. So we created this helpful handout.

    CareOregon Connect to Care

Our Connect to Care team helps connect the community to the health care system and answers member questions about CareOregon benefits.

Every month, our team comes out to the communities where members live, all over the Metro area – including Clackamas, Multnomah and Washington counties. We can also walk you, your staff and your organization through a presentation on the basics of Medicaid. To learn more about the services we provide and upcoming events, visit our Connect to Care page, or call our Customer Services at 503-416-4100 or toll-free 800-224-4840.

You can also join the CareOregon Community Advisory Board (COCAB) to share your feedback and help us advocate for increased awareness of health issues and access to care. Find more information on our Community Advisory Board page

We invite you to CareOregon’s Community Partner News, our quarterly newsletter dedicated to the many community health partners serving the Portland tri-county area. Subscribe to the newsletter using the sign-up in the upper right corner of this page.
