Effective October 1st, 2026, dapagliflozin (generic Farxiga) will be available on the OHP formulary without prior authorization. Dapagliflozin will still have a quantity limit of 1 tablet per day and limited to 30 day supplies. Dapagliflozin is replacing Steglatro entirely which will become non-formulary for new starts effective October 1st, 2026. Existing Steglatro users will need to transition to dapagliflozin before January 1st, 2027 and these members will receive a letter notifying them of this change.
